Scheduling Non-Emergency Standard Transportation Services Through Access2Care
Providers should refer to the table below and contact Access2Care to arrange for medically necessary or covered transportation services.
To assist Exclusively Aligned Enrollment (EAE) D-SNP members who have exhausted their Medicare transportation benefit and need a higher mode of transportation (e.g. a wheelchair van), providers must complete a Physician Certification Statement (PCS) form (Health Net (PDF), CalViva Health (PDF), Community Health Plan of Imperial Valley (PDF)) to help members get transportation services through their Medi-Cal plan.
Submit the completed form to the Health Net Care Ride Unit to help members get authorizations for transportation.
