How to Secure Prior Authorization on the Provider Portal

To submit prior authorization requests online:

Health Net
  1. Log in, then go to the appropriate section for prior authorizations, typically found under Patient Registration or Authorizations.
  2. Follow the prompts to enter the required member and service information, then click Submit to complete the prior authorization request.
  1. Log in, then select the applicable line of business from the drop-down menu > Go.
  2. Once you have checked the member’s eligibility, select the member’s blue hyperlinked name > Authorizations.
  3. Complete the required information to request prior authorization, including submitting the appropriate Smart Sheet for which a link can be found under the Authorizations tab.

  • Validation tools.
  • Prior authorization requirements lists.
  • Inpatient and outpatient forms.

Note: The Health Net and Availity provider portals may not offer the same functions.
