Health Net partners with ModivCare™ (formerly LogistiCare) to provide routine transportation services with no charge to Health Net members enrolled in Health Net Ruby Select, Health Net Gold Select, Health Net Jade, Health Net Seniority Plus Amber I, Health Net Seniority Plus Amber II, Health Net Seniority Plus Amber II Premier, Health Net Seniority Plus Sapphire, and Health Net Seniority Plus Sapphire Premier MA plans. The routine transportation benefit consists of:

  • Health Net Ruby Select: 40 one-way trips to plan-approved locations per member per calendar year.
  • Health Net Amber II Premier and Health Net Seniority Plus Sapphire Premier plans: 40 one-way trips to plan-approved locations per member per calendar year.
  • Health Net Jade C-SNP plan (San Diego, San Francisco, Kern, LA and Orange): 50 one-way trips to plan-approved locations per member per calendar year.
  • Health Net Jade C-SNP plan (Fresno): 40 one-way trips to plan-approved locations per member per calendar year.
  • Health Net Seniority Plus Amber II plan: 30 one-way trips to plan-approved locations per member per calendar year.
  • Health Net Seniority Plus Amber I plan: 50 one-way trips to plan-approved locations per member per calendar year.
  • Health Net Gold Select and Health Net Seniority Plus Sapphire plans: 20 one-way trips to plan-approved locations per member per calendar year.
  • Curb-to-curb or door-to-door service (depending on the member's needs).
  • Rideshare, taxi, standard passenger vehicle, minivan, and wheelchair van transportation.
  • Service to and from medical appointments.
  • No limitation on mileage within the service area.
  • Inclusion of one family member or caretaker on the transport at no additional cost.
  • Member may ask the driver to stop at a pharmacy, radiology provider or laboratory facility from a physician's office (not counted as a separate trip).

ModivCare's business hours for scheduled routine reservations are Monday through Friday, 7:00 a.m. to 6:00 p.m. Pacific time. Transportation services require at least 48 hours' advance notice, and wheelchair vans require at least 72 hours' advance notice. ModivCare is available 24 hours a day, seven days a week to schedule trips for both urgent care needs and immediate transportation needs.
