Community Supports Data/Reports From the Community Supports Provider to Health Net
Community Supports (CS) providers are responsible to submit required reports to Health Net. Required CS provider reports include, but are not limited to the following:
- Monthly CS provider reporting: Community Support Provider Return Transmission File (CSPRTF). Providers must report back one time for both files and must be submitted by the 5th of the following month. If the provider missed the submission time frame, CS providers will have to wait until the following month to resubmit.
- Quarterly submission of Staffing and Capacity Reports.
