Opt Out Provider

A provider who has opted out of Medicare has terminated their Part B contract with Medicare by submitting a valid affidavit to the local Medicare carrier. Affidavits are valid for two years. After the two-year period has expired, the provider may elect to return to Medicare or opt out again. Medicare Advantage with Prescription Drug (Part D) PPO Violet plan members who access care from an opt-out provider sign a private contract with the provider, which advises the member that the provider has opted out of Medicare and neither Medicare nor a MA plan pays any portion of the claim. Services received from a provider that has opted out of Medicare are not covered.
