Provider Type

  • Physicians and Practitioners
  • Ancillary
  • Hospitals

The following information does not apply to Individual & Family Plan (IFP) members.

PPO members may seek care at any time from any PPO participating behavioral health or substance abuse specialist. Physicians may also decide that it is necessary for a member to receive treatment from a behavioral health or substance abuse specialist and should refer the member to a participating specialist when possible.

Health Net does not require authorization for routine in-network outpatient services. Covered routine outpatient services include, but are not limited to:

  • Psychiatric diagnostic interview
  • Individual therapy
  • Family therapy
  • Group therapy
  • Medication management

Psychological and neuropsychological testing are covered services in most benefit plans. Prior authorization is required for psychological and neuropsychological testing. Prior authorization is also required for Applied Behavior Analysis (ABA) Services.

Authorization requests can be submitted by calling the Health Net Behavioral Health call center and obtaining an authorization request form, which can subsequently be submitted by fax or email. For additional information refer to the Prior Authorization section.

To determine whether a behavioral health or substance abuse specialist is a PPO participating provider, physicians can call the Health Net Provider Services Department .
