Human growth hormone (HGH) and antihemophilic factors for Food and Drug Administration (FDA)-approved indications are covered. For participating physician groups (PPGs), HGH is defined as a self-injectable medication under most Provider Participation Agreements (PPAs). Refer to the Medicare Part D Formulary or Cal MediConnect Formulary for HGH and antihemophilic factors.
HGH must be obtained through Pharmacy Services. Antihemophilic factors may be obtained through a Health Net participating specialty pharmacy.
