21-697m Use the New Fax Number for Transplant Prior Auth Requests

This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, and Ancillary providers.

This information applies to Medi-Cal in Fresno, Kings and Madera counties.

Save the new number for future transplant requests

Prior authorization for transplant services is required. You can submit your requests by fax for elective and urgent services.

Below is the new fax number to use for transplant requests:

For all other services, go to the Provider Library to ensure you fax requests to the correct number.

You must also immediately inform Health Net, on behalf of CalViva Health, when there is a request for investigational or experimental treatment.
