This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, and Ancillary providers.
For Medi-Cal, this information applies to Kern, Los Angeles, Sacramento, San Diego, San Joaquin, Stanislaus, and Tulare counties.
Save the new numbers for future transplant requests
Prior authorization for transplant services is required. You can submit your requests by fax for elective and urgent services.
Below are the new fax numbers to use for transplant requests listed by line of business.

For all other services, go to the Provider Library to ensure you fax requests to the correct number.
You must also immediately inform Health Net when there is a request for investigational or experimental treatment.
