26-990 Changes to Community Supports Services, Effective January 1, 2027

Review impacted members and plan for care transitions

Beginning January 1, 2027, the following Community Supports (CS) services will no longer be available. Providers serving members who receive these services must review affected members and plan for care transitions, as appropriate.

Services ending January 1, 2027

The following CS services will end on January 1, 2027:

  • Assisted Living Facility Transitions
  • Personal Care and Homemaker Services
  • Short-Term Post-Hospitalization Housing

Provider actions

  1. Identify members currently receiving any of the affected services.
  2. Continue providing approved services through each member’s authorized service period, not to extend past December 31, 2026.
  3. Coordinate transition planning for affected members, as appropriate.

This information applies to Medi-Cal in Fresno, Kings and Madera counties.
