Record outreach attempts, including unsuccessful contacts, to help meet DHCS audit and documentation requirements

Complete and accurate documentation of Initial Health Appointment (IHA) outreach activities helps support Department of Health Care Services (DHCS) audit requirements. Recording all outreach attempts in Cozeva®, including unsuccessful contact attempts, helps demonstrate that outreach efforts were made to eligible patients and supports audit readiness.

What you must do

Primary care physicians must:

  • Record IHA outreach attempts in Cozeva, when appropriate.
  • Document unsuccessful outreach attempts when a patient cannot be reached.
  • Document completed IHAs for eligible patients.
