Date: 07/28/26

Paper attestations will no longer be accepted, effective August 1, 2026

All new contracting Medi-Cal physicians, practitioners and other providers must complete onboarding training and submit an online attestation after training is complete.

What you need to do

To help avoid delays in Medi-Cal contract activation:

Complete the required onboarding training.

Access the online attestation form at Health Net:

  • Select Provider Training Webinars and Other Provider-Related Resources.
  • Select New Providers Onboarding Materials.
  • Select Health Net Medi-Cal New Provider Resources.
  • Select Attestation of New Provider Training (link will be available on August 1, 2026).

Submit the appropriate online attestation after training is complete using the same form for both submission types:

  • Individual: Submit for one provider.
  • Group (PPG): Submit for multiple providers.
