Annual CMS training for providers treating SNP members
If you care for patients enrolled in a Special Needs Plan (SNP), you must complete Model of Care training required by the Centers for Medicare & Medicaid Services (CMS).
This training applies to providers who serve:
- Dual-Eligible Special Needs Plan (D-SNP) members (patients enrolled in both Medicare and Medicaid).
- Chronic Condition Special Needs Plan (C-SNP) members.
Completing this training helps you understand how care is coordinated and delivered for these members.
What you need to do
- Complete the Model of Care training by December 31, 2026.
- Submit a voluntary attestation after completing the training (available at the end of the course). The attestation is for recordkeeping and supports audit readiness.
