26-811 New Requirement: Submit Medi-Cal Provider Onboarding Training Attestation Online

The Department of Health Care Services (DHCS) requires all new Medi-Cal providers to complete onboarding training and submit an online attestation. These requirements apply to all Medi-Cal provider contracts with an effective date on or after August 1, 2026.
Providers and provider groups must submit an online attestation for each provider who completes the onboarding training to ensure timely Medi‑Cal contract activation.
Completing the online attestation form is quick and easy. You can copy and paste information from other sources to save time. You may save the form for your records or print a copy for reference. Printed forms do not need to be submitted to Health Net.
Who must complete the training
- All new providers joining your medical group under a Medi-Cal agreement.
- Onboarding training must be completed prior to or within 30 calendar days of the Medi-Cal contract activation date.
How to submit your online attestation
To confirm that the required onboarding training is complete, fill out the Attestation of New Provider Training form.
Use the same form for both individual and group submissions. Follow the steps below and choose the option that applies to you.
1 Enter the attestor’s information and select Next.
2 Select the attestation type that applies to your submission:
- Individual: Submitting for one provider.
- Group (PPG): Submitting for multiple providers. Have the following information ready for each provider:
- Full name
- Individual National Provider Identifier (NPI)
- Date the onboarding training was completed
- Provider email address
