26-745 Action Required: Update and Confirm Your Provider Directory Information to Avoid Removal

This clarification includes required submission options missing from May 15, 2026, update
On May 15, 2026, Health Net shared provider update 26-646, Action Required: Keep Your Provider Directory Information Current, outlining the requirements to maintain accurate provider directory information to comply with California Department of Managed Health Care regulations.
This communication provides additional instructions that were not included in update 26-646, including how to submit updates to your demographic information and how to complete required validations.
For Community Supports (CS) or Enhancement Care Management (ECM) providers, this communication applies if you also contract for other lines of business besides Medi-Cal.
What you need to do
To remain listed in the provider directory, you must:
- Review, update and confirm your information regularly.
- Submit updates using one of the approved methods below.
- Respond to validation requests, including quarterly reminders.
Consequences of non-compliance
Physicians, practitioners and other providers with inaccurate or unconfirmed information may be removed from the provider directory.
How to submit updates
Choose one of the following options:
1 Symphony Provider Directory
- Receive quarterly reminders to validate and attest to your information.
- Recommended for participating physician groups (PPGs) – automated updates are available.
- Request access at Symphony Provider Directory Team.
2 Availity Provider Data Management
- Available to directly contracting physicians and practitioners, hospitals, and ancillary providers.
- Register and submit updates at no cost at Portal Registration.
3 Online Demographic Update Form
- Submit updates directly online.
- Access the form at Demographic Update Forms.
- Updates go to the Provider Data Coordination team for processing.
Important reminder for providers contracting with a PPG
- Report all demographic changes to your PPG.
- Do not submit changes directly to Health Net unless instructed.
- Your PPG is responsible for submitting updates to Health Net.
