26-726 Expanded DMHC Notification Requirements for PPG and Hospital Network Terminations

New rules expand the types of network terminations that require earlier notification to PPGs and hospitals
Participating Provider Groups (PPGs) and hospitals are required to notify Health Net, on behalf of Community Health Plan of Imperial Valley, as soon as a network termination is known or anticipated—and pursuant to the requirements of your Provider Participation Agreement (PPA) before the effective date—to meet updated California Department of Managed Health Care (DMHC) requirements effective January 1, 2026.
These requirements now extend to additional termination types, including partial (including loss of service line), non‑renewal, constructive, novation, and assignment‑related terminations
What you must do
- Notify your assigned Provider Network Management contact immediately when a termination is planned or expected.
- Submit notification pursuant to the requirements of your PPA before the termination effective date.
- Provide complete and accurate information to allow sufficient time for regulatory review.
When and where to send notifications
- Send notifications to your assigned Provider Network Management contact.
- Notification is required as soon as a termination is known or anticipated, including:
- Planned non-renewals.
- Early terminations.
- Include termination details and effective dates to support timely review and approval.
Information required
Include the following with your notification:
- Termination notice and effective date.
- Extension notice (if applicable).
- Continuity of care approach and impacted members.
- Scope and impact of the termination, including affected lines of business.
DMHC approval required for hospital terminations
A terminating hospital may not be removed from the network until DMHC approval of the required Block Transfer Filing has been obtained.
Compliance risk
Late or incomplete submissions will result in corrective action and will impact delegation status.
