26-538 Speed Up Housing Support Requests with the Housing Support Plan Template

Use the template to help avoid delays, reduce denials and ensure your request includes all required documentation
The Housing Support Plan (HSP) (PDF) Template is a new, optional tool available to housing providers. You may use the HSP Template or submit your own version. This new template is provided for convenience only and does not change existing requirements or processes.
When to use the HSP Template
Use the HSP Template when submitting requests for:
- Housing Deposits
- Transitional Rent
- Authorization extensions for:
- Housing Navigation
- Housing Tenancy and Sustaining Services
Important: Carefully review the submission instructions. Requests submitted without a completed HSP, when required, will be automatically denied.
Submission requirements
To avoid processing delays or denials, follow all current submission guidelines:
- Submit a prior authorization request and include a completed HSP when required.
- Keep a copy of the HSP in the member’s file for audit purposes.
- Include all required supporting documents with your request.
Incomplete or missing documentation may result in denial.
Where to find the HSP template
- Go to the CalAIM Resources for Providers page.
- Select Forms & Tools.
- Choose Community Supports (CS).
- Download the Housing Support Plan Template under Authorization/Referral Forms.
