26-443 Updated Assisted Living Transition Requirements

Assisted Living Waiver facilities required for transitions in select counties
Effective June 17, 2026, the Assisted Living Transition authorization documents will include updated guidance on transition limitations. For transitions occurring for members residing in the counties listed below, you are required to use an Assisted Living Waiver (ALW) program participating facility:
- Fresno
- Los Angeles
- Sacramento
- San Joaquin
For all other California counties, transitions to non-waiver facilities are allowed.
A list of all California Department of Health Care Services waiver-approved assisted living facilities is available at Assisted Living Waiver Program Participating Facilities.
What you need to do
You must ensure that all information submitted on the authorization form is complete and accurate, including clear documentation of the name, physical address and county in which the assisted living facility is located.

This information applies to Medi-Cal in Fresno, Kings and Madera counties.
