An enhanced resource that simplifies referrals
Based on feedback from our physician, practitioner and other provider partners, we’ve updated and renamed our contracted provider resource for palliative care to better reflect available services.
What’s new in the updated list
The Palliative and Hospice Care Contracted Provider List now makes it easier for you to quickly identify the most appropriate contracted resources for palliative and hospice care referrals. Updates include:
- Provider categories: Separated contracted providers into preferred and secondary lists.
- Hospice services: Identification of contracted facilities that also provide hospice care.
- Service areas: Counties covered by each provider.
- Fax numbers: Included to support streamlined referrals.
- Website links: Direct link to provider websites.
How to access and use the updated list
- Go to the Provider Library.
- Select Medi-Cal, then navigate to Forms and References.
- Begin using the updated Palliative and Hospice Care Contracted Provider List immediately for all palliative and hospice care referral needs.
