Share by April 7 whether you provide gender-affirming services to support accurate provider directories

The Department of Health Care Services requires health plans to include specific information in their provider directories related to gender-affirming services.

What we need you to confirm

To meet this requirement, please confirm:  

  • Whether your office offers and has provided gender‑affirming services,
  • The specific gender‑affirming services you provide,
  • If you would like this information displayed in the provider directory.

How this information is collected

To meet this requirement, we have launched a brief survey to collect and confirm this information.  

Complete the survey by April 7, 2026

The survey includes five questions and takes less than 10 minutes to complete. Please have your NPI number available before you begin:

Go to 2026 Health Net Gender-Affirming Services Survey to complete the survey. The survey must be completed by the physician, practitioner or other provider and/or by the physician’s, practitioner’s or other provider’s authorized representative.

Gender-affirming services

Gender-affirming services include, but are not limited to:

  • Feminizing genitoplasty  
  • Feminizing mammoplasty
  • Gender-affirming gynecological care       
  • Gender-confirming facial surgery
  • Hormone therapy related to gender dysphoria or intersex conditions      
  • Hysterectomy      
  • Male chest reconstruction
  • Mastectomy
  • Metoidioplasty
  • Oophorectomy
  • Penectomy, orchiectomy
  • Phalloplasty
  • Scrotoplasty
  • Voice masculinization or feminization
  • Voice therapy related to gender dysphoria or intersex condition
