26-085 Important Update to Medicare Add-On Codes G2211 and G2212

These codes will no longer be accepted starting April 1, 2026
Effective April 1, 2026, Health Net will no longer accept or reimburse CPT codes G2211 and G2212.

- G2211: Add-on code to describe the complexity associated with an ongoing, longitudinal practitioner–patient relationship during an office or outpatient visit.
- G2212: Add-on code to report prolonged office or outpatient evaluation and management (E/M) services beyond the maximum time of a primary E/M code, billed in 15‑minute increments.
Billing guidance, effective April 1, 2026
Providers should discontinue use of G2211 and G2212 for dates of service on or after April 1, 2026. Instead, bill the standard E/M codes listed below, as appropriate:
- 99202 – 99205
- 99211 – 99215
These codes should be used to accurately to reflect the level and duration of E/M services provided.
