26-039 Updated Personal Care and Homemaker Authorization Form: Requests Are Limited to 10 Hours Daily

The daily cap for Personal Care and Homemaker Services is effective March 25, 2026

Effective March 25, 2026, the Personal Care and Homemaker Services Authorization Form has been updated to limit provider requests to a maximum of 10 hours per day (of In-Home Supportive Services and Personal Care and Homemaker Services hours combined) for eligible members. Any request exceeding this 10-hour daily limit will be denied.

Personal Care and Homemaker Services supports members with essential daily living and instrumental activities. If the member requires more than 10 hours a day of assistance with activities of daily living, this service may not be appropriate for the level of care needs. Members with higher care needs should be referred to Enhanced Care Management or their primary care provider for assessment.
