Post-Hospitalization Housing (STPHH) using the updated guides and forms

STPHH provides support to members who are exiting an institution and experiencing or at risk of being homeless with the opportunity to continue their medical/behavioral health/substance use disorder recovery immediately after exiting the institution.

STPHH benefits provide members with ongoing supports necessary for recuperation and recovery, such as regaining the ability to perform activities of daily living, receiving necessary medical/behavioral health/substance use disorder care, receiving case management and access to other housing supports.

Recent updates to STPHH authorization guides and referral forms

The authorization guides and referral forms for STPHH have been updated to reflect the latest program requirements. Key updates include:

This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, Ancillary Providers, Community Supports (CS) Providers, Enhanced Care Management (ECM) Providers, and Behavioral Health Providers.

This information applies to Medi-Cal in Fresno, Kings and Madera counties.
