The Health Outcomes Survey is underway until November 2025

The Medicare Health Outcomes Survey (HOS) is currently in progress and will continue through November 2025. This important survey gathers feedback directly from Medicare patients about their health and the care they receive over time.

Physicians and other providers play a critical role in shaping HOS results. Patients’ perceptions about their health are influenced by the quality of communication and care they receive. These results also impact the Centers for Medicare & Medicaid Services Star Ratings, which reflect the performance of health plans.
