25-567 Updates to Housing Deposit Authorization Guide and Referral Form

Changes take effect August 21, 2025 to help you assess and refer members for services to secure permanent housing
Effective August 21, 2025, Health Net, on behalf of Community Health Plan of Imperial Valley, is updating its Housing Deposit authorization guide and referral form.
Housing Deposits help individuals get one-time support to move into permanent housing like paying for deposits, utilities and essential items needed to set up a home.
What’s new?
Referral form changes
- Requested Items – Items are now grouped into categories. Please note, some items may have cost limits. The maximum allowance for all items (including taxes) remains unchanged and must not exceed $6,000.
Authorization guide changes
- Upon move-in, Housing Deposit items must be requested within 30 calendar days of the move-in date listed on the lease agreement.

The Housing Deposit authorization guide and referral form are available on the CalAIM resources page > Community Supports > Forms & Tools.
The updated guide and form that reflect the changes described in this update will be available online as of August 21, 2025.
