25-533 Do You Want Your Gender-Affirming Services Displayed in Provider Directories?

Please complete a brief survey about gender-affirming services

Effective February 14, 2025, the Department of Health Care Services requires health plans to include information within their provider directories that:

  • Identifies which of their in-network providers have confirmed they offer and have provided gender-affirming services and
  • In their directories, includes a list of the gender-affirming services provided.

Health Net, on behalf of Community Health Plan of Imperial Valley, has launched a brief survey to collect and confirm the gender-affirming services offered and provided by our contracted providers to display in directories.

Please complete survey by June 13, 2025

The survey has five questions and can be completed in less than 10 minutes. Have your national provider identifier number, taxpayer identification number and Health Net assigned ID available. Go to Health Net Gender-Affirming Services to complete the survey. The survey should be completed by the physician and other providers and/or by the physician’s or other provider’s representative. Respond to the survey by indicating:

  • Whether your office offers and has provided gender-affirming services.
  • The gender-affirming service(s) that you provide.
  • Whether or not you want this information displayed in the provider directories.

Gender-affirming services

Gender-affirming services include, but are not limited to:

  • Feminizing genitoplasty
  • Feminizing mammoplasty
  • Gender-affirming gynecological care
  • Gender-confirming facial surgery
  • Hormone therapy related to gender dysphoria or intersex conditions
  • Hysterectomy
  • Male chest reconstruction
  • Mastectomy
  • Metoidioplasty
  • Oophorectomy
  • Penectomy, orchiectomy
  • Phalloplasty
  • Scrotoplasty
  • Voice masculinization or feminization
  • Voice therapy related to gender dysphoria or intersex conditions

Providers are encouraged to access the provider portal for real-time information, including eligibility verification, claims status, prior authorization status, plan summaries, and more.

This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, Ancillary Providers, Community Supports (CS) Providers, Enhanced Care Management (ECM) Providers, and Behavioral Health Providers.
