25-531 Do You Want Your Gender-Affirming Services Displayed in Provider Directories?
Date: 06/05/25
Please complete a brief survey about gender-affirming services
Effective February 14, 2025, the Department of Health Care Services requires health plans to include information within their provider directories that:
- Identifies which of their in-network providers have confirmed they offer and have provided gender-affirming services and
- In their directories, includes a list of the gender-affirming services provided.
Health Net has launched a brief survey to collect and confirm the gender-affirming services offered and provided by our contracted providers to display in directories.
Please complete survey by June 13, 2025
The survey has five questions and can be completed in less than 10 minutes. Have your national provider identifier number, taxpayer identification number and Health Net assigned ID available. Go to Health Net Gender-Affirming Services to complete the survey. The survey should be completed by the physician and other providers and/or by the physician’s or other provider’s representative. Respond to the survey by indicating:
- Whether your office offers and has provided gender-affirming services.
