Health Net is excited to announce the launch of an initiative approved by the Department of Health Care Services (DHCS). This program enables our care managers to connect with Medi-Cal members directly through text messaging within seven days post discharge. Designed to address member communication preferences, this approach makes it easier for patients to access care and support post discharge.
Members request texts vs. calls
Based on direct member feedback, we’ve made it possible for our Medi-Cal members to receive important information on follow-up care via secure text messaging. This proactive connection helps ensure smoother transitions from hospital care to home, improving overall outcomes and satisfaction.
How you can support this initiative
Your partnership is essential to making this initiative a success. We kindly ask your discharge team to communicate this initiative to Medi-Cal members before they leave the hospital. Here’s how your team can help:
- Inform members: Share that they will receive a secure text from Health Net within seven days of discharge.
- Provide clarity: Ensure members understand the text is from Health Net, encouraging them to engage instead of dismissing it as spam.
- Include messaging: Incorporate the messaging below in Health Net’s member discharge paperwork, ensuring they are well-informed.
