25-295 ADT Reports Change From Excel to CSV Format Effective June 6, 2025

The CSV format will include more data elements and improve data exchange requirements

Effective June 6, 2025, Health Net, on behalf of Community Health Plan of Imperial Valley, will implement  updates to provider admit, discharge and transfer (ADT) reports to help providers and improve the reporting data results. This change will enable physicians and other providers currently not receiving ADT reports to sign up and receive these reports in real time. ADT reports include inpatient (IP)/emergency room (ER) and alternate contact information.

Improvements to ADT reports

The upcoming changes are as follows:

  • Format of report will change from an Excel file to a comma-separated values (CSV) file for data exchange.
  • New fields will be added to the reports to alert/flag sensitive measures.
  • The format change will significantly improve the reliability of the reports and ensure they are consistently formatted across providers. These changes should help providers receive the reports in a timely manner.

Benefits of ADT reports

The reports help providers with the following:

  • Alert providers about a patient's status change.
  • Ensure timely care coordination.
  • Ensure compliance with data exchange requirements.
  • Provide outreach and case management to members discharged from acute care.
  • Follow up after an emergency department visit for mental illness (FUM).
  • Follow up after an emergency department visit for alcohol and other drug abuse or dependence (FUA).
  • Provides information on care gap opportunities, as well as insights into physicians’ and other providers’ performance on key quality measures compared to national benchmarks and the Health Net network.
  • The data are also used to engage with members to help improve member health outcomes.

How to access the reports

Health Net posts the ADT IP/ER and alternate contact reports daily on Cozeva®. The reports are also available through an SFTP site.
