The contact information to submit written correspondence related to your contract agreement with Network Providers, LLC, has been updated. As required under Section 7.111 of the provider contract agreement, you can submit information by mail, fax or email to:

Important notice requirements

If you need to provide notices regarding contract breach, term changes, termination, or renewal, please follow these guidelines for submission:

  • Written notice via U.S. mail
    • Notices sent via mail will be considered delivered five (5) days after they are deposited in the U.S. mail with proper postage.
  • Hand delivery, overnight courier or fax
    • Notices sent via these methods will be deemed delivered upon official documentation of receipt.

Section 7.11 – Network Provider, LLC contract
7.11 Notice. Notices regarding the breach, term, termination or renewal of this Agreement shall be given in writing in accordance with this Section 7.11 and shall be deemed given five (5) days following deposit in the U.S. mail, postage prepaid. If sent by hand delivery, overnight courier, or facsimile, notices shall be deemed given upon documentation of delivery. All notices shall be addressed as follows:

The addresses to which notices are to be sent may be changed by written notice given in accordance with this Section. Notwithstanding the previous paragraph, NPLLC may provide all other notices by electronic mail, through its provider newsletter, or on its provider website.
