Follow mandatory screening and anticipatory guidance documentation requirements to keep children safe

Why it matters

Lead exposure can cause irreversible harm to a child’s health and development. The American Academy of Pediatrics and the Centers for Disease Control and Prevention emphasize that no level of lead exposure is considered safe—even low levels can impair cognition and behavior. To protect children and remain compliant, providers must follow screening and documentation requirements.

Anticipatory guidance

Providers must give anticipatory guidance on lead exposure at every periodic health assessment and document counseling on reducing risks and the importance of timely testing in the medical record.

  • Discuss lead exposure risks at every periodic health assessment.
  • Document guidance in the member’s medical record.
  • Member Resources: Order member educational materials from your County Childhood Lead Poisoning Prevention Program or download from the California Department of Public Health (CDPH) Childhood Lead Poisoning Prevention Branch.

Blood lead screenings

Providers are required to order lead blood screenings at these key points:

  • Ages 1 and 2 years: Mandatory screening at both milestones.
  • Missed test: If a child under 6 has not received the required screening.
  • New risk factors: When circumstances increase exposure risk.
  • Parent request: If medically indicated and requested by a parent or guardian.

Additional requirements

  • Refugees: Screen upon arrival and at specified intervals.
  • Refusal: If a parent or guardian declines screening, obtain a signed voluntary refusal statement and document it.
