25-1177m Updated Authorization Guides: Personal Care and Homemaker Services and Day Habilitation Services

Aligning services with the DHCS standards for improved care

To meet current Department of Health Care Services (DHCS) requirements, we made minor revisions to the general service overview in the authorization guides listed below. Eligibility and service offerings remain unchanged.

Key updates for Day Habilitation Services

  • Authorization periods: Initial authorizations and extensions are now available for up to 90 days, based on member needs.
  • Unit of service: Authorization unit has changed from per diem to per hour.
  • New referral form: Use the Day Habilitation Referral Form (PDF) to refer eligible members for services.

For Medi-Cal, this information applies to Amador, Calaveras, Inyo, Los Angeles, Mono, Sacramento, San Joaquin, Stanislaus, Tulare and Tuolumne counties.
