Effective February 20, 2026, CPT code 0449U is no longer a covered benefit. This change is based on the following:
- Limited use: CPT code 0449U is a Proprietary Laboratory Analysis (PLA) code used exclusively by a single provider.
- In-network alternatives available: Equivalent services are available through in-network laboratories such as Quest Diagnostics and LabCorp.
What is CPT code 0449U?
CPT code 0449U refers to the UNITY Carrier Screen™, a PLA developed by BillionToOne Laboratory. This test uses next-generation sequencing to screen for inherited conditions by analyzing five genes: CFTR, SMN1, HBB, HBA1 and HBA2.

To help patients avoid unnecessary out-of-pocket costs, please refer them to in-network labs like Quest Diagnostics and LabCorp for services previously associated with CPT code 0449U.
