25-1085 New Policy Expands Access to Hepatitis C Treatment

Full-course dispensing now allowed as of October 1, 2025

Effective October 1, 2025, the California Department of Health Care Services (DHCS) has removed the 28-day supply limitation for Hepatitis C treatments under Medi-Cal RX. Physicians and other providers may now dispense up to a 12-week supply per fill -- supporting uninterrupted care and better health outcomes.

Why this matters

This policy change is backed by strong clinical evidence and strives to reduce barriers to care for Medi-Cal members.

  • High completion rates among Medi-Cal members
    • Glecaprevir/Pibrentasvir: 89% completion rate
    • Sofosbuvir/Velpatasvir: 83% completion rate

  • MINMON study confirms safety and effectiveness
    • 89% of participants took 100% of their medication over 12 weeks
    • 95% achieved sustained virologic response (SVR)
  • Expanded dispensing supports justice-involved populations
    • Enables treatment continuity for individuals transitioning from incarceration to the community
    • Reduces risk of treatment interruption and need for complex regimens

What providers need to know
