25-1025 Community or Home Transition Services Ending January 1, 2026

SNFs and LTSS teams will continue member support
Starting January 1, 2026, Health Net will no longer offer the Community Support service known as Community or Home Transition Services (formally “Community Transition Services/Nursing Facility Transition to a Home”).
This change is not expected to impact members, as skilled nursing facilities will continue to provide similar transition support services.
What services will continue
Health Net will still offer:
- Assisted Living Facility Transitions services– Supports transitions from nursing facility to home-like, community settings such as a residential care facility for the elderly or adult residential care facility, and/or to help prevent nursing facility admissions.
- Post-transition support from long-term services and supports (LTSS) care managers – Members will continue to receive follow-up and coordination from the LTSS care management team.
Additional information
Relevant sections of Health Net’s provider operations manuals have been revised to reflect the information contained in this update as applicable. Provider operations manuals are available electronically in the Provider Library on Health Net’s provider portal.
