24-1032 Reminder: Verify Provider Demographic Data

Let us know about changes before they take effect

To ensure California Department of Corrections and Rehabilitation (CDCR) has access to accurate information when selecting providers, Prison Health Care Provider Network (PHCPN) physicians, hospitals and ancillary providers are required to provide advance notification to Health Net of California, Inc. (Health Net) when they have changes to their demographic information. On a monthly basis, physicians, hospitals and ancillary providers should validate that their demographic information is reflected correctly on the Health Net website under ProviderSearch.

Demographic information

Demographic data for physicians, hospitals and ancillary providers includes the following:

  • Name
  • Address
  • Phone number
  • Fax number
  • Office hours
  • Languages other than English spoken by the physician(s)
  • Handicap accessibility status for parking (P), exterior building (EB), interior building (IB), restroom (R), exam room (ER) and exam table/scale (T). If accessibility is not yes to all, then indicate no.

Notification and maintenance requirements

Physicians, hospitals and ancillary providers directly contracting with Prison Health Care Provider Network must notify Health Net of changes by reaching out to their provider network administrator or sending an email. Physicians, hospitals and ancillary providers must have policies in place that establish and implement processes to collect, maintain and submit their provider demographic changes to Health Net on a real-time basis. Real-time is within 30 days, as defined by the Centers for Medicare & Medicaid Services.
