How to handle Molina member requests to transfer back to Health Net

We have received numerous inquiries from providers in Los Angeles County regarding members who were transitioned to Molina as their Medi-Cal plan effective January 1, 2024, but have expressed a desire to continue with Health Net as their preferred Medi-Cal plan.

If your patient has been transitioned to Molina but wishes to stay with Health Net, please complete the Request for PCP/PPG Change Form (English (PDF) and Spanish (PDF)) and check the

Health Net box on top of the form. It is crucial that the form is completed while the member is in your office.

Once completed, please submit the form to Health Net Medi-Cal Member Services via the fax or email provided on the form. Make sure to include a future effective date on the form and ensure it is acknowledged and signed by the member. Please allow for a 15 business-day turn-around time for processing.
