Access the flyer in the Provider Library at Avoid Processing Delays for Prior Authorization Requests with These Guidelines (PDF). 

PA request forms are also found in the Provider Library > Medi-Cal > Forms and References. Then select either Inpatient or Outpatient California Medi-Cal Prior Authorization.
