Use the updated referral form for Fit Families for Life

The Fit Families for Life weight management program workout DVD has been replaced with online video clips. The clips can be accessed on the CalViva Health website by clicking on the YouTube icon at the top right of the main page or select CalViva Health - Health Education.

The weight management resources are:

  • Fit Families for Life! – Home Edition (FFFL): FFFL is a five-week, self-paced, home-based family intervention resource. It guides members toward better food choices and increased physical activity,

           and

  • Healthy Habits for Healthy People (HHHP): HHHP is a self-paced intervention resource for older adults aimed to improve food choices and physical activity.

What’s included

Both resources offer:

  • Workbook
  • Exercise stretch band
  • Cookbook  
  • Online workouts with easy-to-follow exercise videos designed for different levels of physical activity.

You can help CalViva Health members get FFFL or HHHP resources by faxing or emailing the provider referral form to our Health Education Department at:
