Take this survey to help employers of community health workers (CHWs)/promotoras/community health representatives (CHRs) get ready for operationalization and reimbursement of the Department of Health Care Services’ new care transformation initiatives. These include the new Medi-Cal coverage of CHW/promotora/CHR worker services.

The survey takes only 10 minutes. Take the survey at Community Health Worker / Promotora / Representative (CHW/P/R) Employer Survey. The survey ends March 31, 2023.

Physicians and other providers who employ CHWs/promotoras/CHRs are encouraged to complete the survey.

The goal of the survey is to learn more about employers of CHWs/promotoras/CHRs and align with broader statewide efforts to understand the CHW/promotora/CHR employer landscape. The CHW/promotora/CHR workforce is critical to delivering effective, equitable care and advancing health equity for historically marginalized communities.

Employers who take part will enter a raffle. One winner will receive a $500 gift card and two winners will each receive a $250 gift card. All participants will receive aggregate results of the survey.
