Use the updated referral form for Fit Families for Life – Be in Charge!SM
The Fit Families for Life – Be in Charge!SM weight management program workout DVD has been replaced with online video clips. The clips can be accessed on the Health Net* website> Health & Wellness > Online Fun Fitness and on our Health Net YouTube channel.
The weight management programs are:
Fit Families for Life! – Home Edition (FFFL): FFFL is a five-week, self-paced, home-based family intervention resource. It guides members toward better food choices and increased physical activity,
and
Healthy Habits for Healthy People (HHHP): HHHP is a self-paced intervention resource for older adults aimed to improve food choices and physical activity.
What’s included with the programs
Both programs offer:
- Workbook
- Exercise stretch band
- Cookbook
- Online workouts with easy-to-follow exercise videos designed for different levels of physical activity.
How to refer members
You can refer Medi-Cal members to the FFFL or HHHP programs by faxing or emailing the provider referral form to our Health Education Department at:
