This information applies to Physicians, Hospitals, and Ancillary providers.

California inmates in reentry programs can access health care through the Prison Health Care Provider Network

Effective July 1, 2022, the Prison Health Care Provider Network (PHCPN) is available to the California Department of Corrections and Rehabilitation’s Reentry Programs. Providers contracted with Network Providers, LLC (NPLLC) and participating in the PHCPN will provide medical services to the reentry population. The “reentry” population are offenders who are eligible to participate in pre-release community programs. These individuals are serving the last part of their sentence in community programs throughout California instead of confinement in state prison.

Claims submission

Providers submit paper claims for services provided to the reentry population to:

Correct Care Integrated Health

P.O. Box 349026

Sacramento, CA 95834-9026

For more claims information, refer to the PHCPN provider manual, available on the Health Net provider website > Provider Manual under Claims and Provider Reimbursement. Covered services are reimbursed at NPLLC contract terms.
