These no-cost health education programs and resources can help you meet the Staying Healthy Assessment requirements.

Help your patients lose weight, stop smoking and live healthier lives. Health Net offers health education programs and resources at no-cost to Medi-Cal members. You can refer patients to these programs and use the resources to meet the California Department of Health Care Services (DHCS) Staying Healthy Assessments’ (SHA) requirements.

  • California Smokers’ Helpline. Help members get no-cost phone counseling and self-help materials to help quit smoking, stop vaping and stay tobacco-free.
  • Diabetes Prevention Program. Help members who are at risk of diabetes prevent this chronic disease. This mobile app connects patients to health coaches, tips and support.   
  • Digital Health Education Programs. Help teens and adults learn about important health topics and resources offered through the T2X social network.
  • Fit Families for Life – Be In Charge! SM  Members and their families can manage their weight. This five-week, home-based nutrition and physical activity program will help.
  • Health Education Materials. Help members get educational materials on asthma, healthy eating, diabetes, immunizations and more.
  • Healthy Hearts, Healthy Lives Program. Help members prevent or manage heart disease. This program includes a toolkit with a tracking journal.
  • myStrength™ Help members stay fit – both mentally and physically. Topics include stress, anxiety, chronic pain, depression and more.
  • Start Smart for Your Baby Pregnancy Program. Pregnant members can get educational materials to help achieve a successful pregnancy, deliver a healthy baby, learn how to breastfeed and care for a baby.

How to refer members and order resources

To refer members to the programs, contact Health Net’s Health Education Department at 800-804-6074. You can also use the Provider Order Form for Health Education Materials to order health education materials. To access the form, log onto the Provider Library, select your line of business and search under Forms and References.

Providers are encouraged to access the provider portal for real-time information, including eligibility verification, claims status, prior authorization status, plan summaries, and more.
