Health Net members have access to a number of wellness resources online and by phone
Your Health Net members have access to a variety of wellness programs related to weight management, stress management, and tobacco cessation online and by phone. Members also have access to their online Health Risk Questionnaire (HRQ), personal health record, healthy discounts, trackers and newsletters.
Members can access the wellness resources by selecting Wellness Center after logging in to the member website for Employer Group Plans or for Individual & Family Plans.
Health Risk Questionnaire
Encourage members to take the 20 minute health risk questionnaire. Questions focus on physical activity, nutrition, emotional health, lifestyle choices (e.g., alcohol, tobacco use and safety), biometric screening results, financial well-being and more. Members will get instant results with recommendations and resources based on their unique health profile.
Online Health Promotion Programs
The six-week, online, health-promotion program process emulates successful 1:1 health coaching, and the programs are personalized to the individual using a whole person approach. Health Promotion Programs include:
- Tobacco Cessation
- Nutrition
- Weight Loss
- Physical Activity
- Stress Management

- Help identify motivators for change, which help the member identify and overcome barriers.
- Set action plans with specific incremental goals to help overcome any existing barriers.
- Work with the member to incorporate positive healthy behaviors in their daily life and improve confidence to sustain long term behavior change.
Wellness Health Coaching Programs include:
- Healthy weight (BMI) maintenance.
- Smoking/tobacco cessation.
- Physical activity.
- Healthy eating/nutrition.
- Managing stress.
Quit for Life
Health Net members can get one-to-one support with a quit coach with the Quit for Life telephonic tobacco-cessation program. The quit coach will work very close with the member to address readiness to change, motivation for quitting, and be a support system for success, as quitting smoking or tobacco is not an easy habit to break. Members will receive four outbound coaching calls and have unlimited inbound calls to their quit coach.
Members will also have access to online learning modules, discussion forums, progress trackers and chat function with online coaches.
By joining the Quit for Life Program, members have access to the below:
- Quitting at their own pace – choose a quit date and prepare to quit.
- Conquer urges to smoke – learn how and when urges to smoke occur and how to cope with them; learn how to manage stress without smoking.
- Use quit medicines so they really work – help selecting an appropriate tobacco aid product, such as over-the-counter or prescription.
- Don’t just quit, become a non-smoker – helping make this quit the last quit, managing weight during the quitting process.
Members can register for the Quit for Life telephonic tobacco cessation program by calling 800-893-5597 to speak to an enrollment specialist and enroll or dial direct at 866-QUIT-4-LIFE (866-784-8454).
Personal Health Record (PHR)
The PHR is part of the wellness program. Members’ PHRs are activated when they access the wellness portal (via trackers, virtual coaching action plan, health assessment). The PHR:
- Stores, tracks and manages health information, tests, and immunizations. PHR also includes diagnosed health conditions and allergies; medications members are taking or have discontinued; procedures, tests, or member exams; and members doctors, hospitals, and pharmacies.
- Helps members learn the state of their health.
- Helps members make sure they are taking medications safely. Members will get an alert if their medications may have harmful effects when taken at the same time.
Members get the most out of a PHR when they keep their records updated over time. They can see and track changes in their health. This can really help for members with a chronic condition, such as heart disease or diabetes.
