21-263 Updates to COVID-19 Vaccine Eligibility Requirements

This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, and Ancillary providers.

For Medi-Cal, this information applies to Kern, Los Angeles, Riverside, Sacramento, San Bernardino, San Diego, San Joaquin, Stanislaus, and Tulare counties.

Effective April 1, more California residents can be vaccinated

NOW VACCINATING

  • Persons ages 65 and older
  • Sector groups
    • Health care workers
    • Long-term care residents
    • Education and childcare
    • Emergency services
    • Food and farming
  • Persons ages 16 and older with one of the following high-risk medical conditions or disabilities are eligible for the COVID-19 vaccine.
    • Cancer, current with weakened immune system.
    • Chronic kidney disease, stage 4 or above.
    • Chronic pulmonary disease, oxygen dependent.
    • Down syndrome.
    • Solid organ transplant, leading to a weakened immune system.
    • Pregnancy.
    • Sickle cell disease.
    • Heart conditions, such as heart failure, coronary artery disease or cardiomyopathies (but not hypertension).
    • Severe obesity (Body Mass Index ≥ 40 kg/m2).
    • Type 2 diabetes mellitus with a hemoglobin A1c level greater than 7.5%.

OR if, as a result of a developmental or other significant high-risk disability, one or more of the following criteria applies:

  • A COVID-19 infection is likely to result in severe life-threatening illness or death.
  • Getting COVID-19 will limit the person’s ability to receive ongoing care or services vital to their well-being and survival.
  • Providing adequate and timely COVID-19 care will be particularly challenging as a result of the individual’s disability.

Proof for high-risk conditions or disabilities

  1. Residents are not required to provide documentation of high-risk medical conditions and/or disabilities.
  2. Residents are required to sign a self-attestation that they meet the criteria for the high-risk medical conditions and/or disabilities outlined above.

AS OF APRIL 1, 2021 - All individuals age 50 or older are eligible for vaccination.

AS OF APRIL 15, 2021 - Every California resident age 16 or older is eligible for vaccination.
