Members can access non-emergency and non-medical transportation services with ModivCare
On January 6, 2021, LogistiCare changed its name to ModivCareTM. This change focuses on the member experience that delivers consistent, positive healthcare outcomes through a tech-enabled healthcare company. ModivCare is Health Net’s capitated preferred provider. Refer below for contacts to arrange for transportation services.
Referral processes, phone numbers, daily support and services all remain the same.
Referrals
Any referral source (participating physician groups (PPGs), hospitals, skilled nursing facilities, etc., not financially at risk for transportation services) is required to contact ModivCare to arrange for transportation services. Using transportation services from any provider other than ModivCare may result in the denial of the claim for which the referral source may be liable.
Dual-risk PPGs and hospitals
As a reminder, PPGs or hospitals who have risk for non-emergency medical transportation (NEMT) in the Division of Financial Responsibility (DOFR) must authorize and arrange NEMT with their transportation provider in a timely manner. Failure to do so will result in Health Net approving and arranging the transportation, and processing a capitation payment deduction.

The website is now modivcare.com. Providers may schedule routine transports with an advance notice of five business days through the website.
