21-029 Updated CalViva Health-Specific Nondiscrimination Notice

This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, and Ancillary providers.

This information applies to Medi-Cal in Fresno, Kings and Madera counties.

Use updated nondiscrimination notice for members

The nondiscrimination notices you send to your members have been updated. Please start using the updated notices immediately.

The below protected classes were added to the nondiscrimination notices1:

  • Ethnic group identification
  • Mental disability
  • Physical disability
  • Medical condition
  • Genetic information

Providers, including case management and utilization management (UM)-delegated providers, are required to include a CalViva Health-specific nondiscrimination notice and taglines with vital documents sent to CalViva Health members (for example, UM denial and delay notices, and claims notices that require member action).

You can find the updated nondiscrimination notice by:

  1. Going to the provider library.
  2. Select Medi-Cal > Forms and References to find the Nondiscrimination Notice (CalViva).

Additional Information

Relevant sections of the provider operations manuals will be revised to reflect the information contained in this update, as applicable. Provider operations manuals are available electronically in the Provider Library, located on the provider website.
