Health Net is looking to award $1.8 million in grants for Federally Qualified Health Centers (FQHCs), Community Based Organizations (CBOs) and clinic consortia in California.

The 2021 Community & Infrastructure Investment Program will provide grants of up to $100,000 per applicant over a 13-month period. The goal of this program is to improve access to quality care and educate residents about Medi-Cal benefits by providing grant funds.

Grant fund uses

These grant funds must be used to:

  • Inform residents of the availability and benefits of health care coverage.
  • Help residents enroll in health care coverage.
  • Assist residents as they navigate the health system after enrollment.
  • Help residents with the coverage renewal process.

Innovation is a key benchmark for these grants. Providers are moving from traditional methods of providing access to care due to the COVID-19 pandemic. A few examples of innovative efforts may include:

  • A mobile app to help consumers find culturally competent care.
  • Investing in grassroots marketing to meet consumers where they live.
  • Employing innovative technology to help consumers navigate a complex health program.

November 30, 2020 submission deadline

We look forward to receiving your completed proposal by November 30, 2020 by 5 p.m. Pacific time (PT), for consideration.

Interested applicants can learn about eligibility requirements, grant request specifications and other key information by reviewing the Community & Infrastructure Investments Enhanced Access Request for Proposal document at theĀ provider website and select News Center at the bottom of the home page.
