20-756 Virtual Meeting to Address COVID-19 Risks for Older Adults and People with Disabilities

Health Net and Anthem Blue Cross, in collaboration with the Department of Health Care Services, are hosting a local meeting to encourage county-level collaboration among home and community-based services (HCBS) and long-term services and supports (LTSS) to address the needs of Medi-Cal beneficiaries during the COVID-19 pandemic.

Find out what resources are available to support Medi-Cal beneficiaries and provide services in the most appropriate setting. This is critical as the number of COVID-19 cases has significantly increased in Central Valley counties over the past several weeks.

Focus

  • Strengthening communication, collaboration and HCBS referrals among local participants, for both Medi-Cal managed care and fee-for-service beneficiaries
  • Strengthening coordination across services and resources and identify appropriate discharge planners
  • Promoting coordination with local HCBS organizations
  • Empowering hospital discharge planners to partner with and transfer patients to the appropriate LTSS provider
