20-478 Use Health Net Preferred Providers Apria, Coram, E-Medical Supplies and LogistiCare

This information applies to Physicians, Participating Physician Groups (PPGs), Hospitals, and Ancillary providers.
Members can take advantage of the better benefits when you refer them to these providers
The following providers are designated preferred providers for Health Net plan members:
- Apria Healthcare® (Apria) – for covered durable medical equipment (DME)
- Coram® – for home infusion services
- E-Medical Supplies – for glucose meters, chemstrips and lancets
- LogistiCare – for non-emergency medical transportation services
When Health Net is financially responsible for services, physicians and participating physician groups (PPGs) must obtain those services from the above preferred providers to be covered. Payment for services not provided by the designated preferred provider is the financial responsibility of the requesting provider or facility.

Apria Healthcare
Refer members to Apria for medically necessary DME and home respiratory services. Apria is the preferred provider for Cal MediConnect plans. DME supplies include, but are not limited to:
- Bandages
- Bilevel positive airway pressure (BiPAP) supplies
- Braces
- Breast feeding pumps supplies
- Breast pumps
- Canes
- Catheters
- Continuous positive airway pressure (CPAP) devices
- Crutches
- Feeding tubes
- Nebulizers
- Negative pressure wound pumps
- Oxygen equipment
- Syringes, not pre-filled
- Walkers
- Wheelchairs
- Whirlpool bath equipment
Coram
Refer members to Coram for home infusion products and services. Coram is the preferred provider for Cal MediConnect plans. Home infusion services include, but are not limited to:
- Aerosolized antibiotics
- Antibiotic, antiviral and antifungal therapies
- Anticoagulant therapy
- Hydration
- Immunoglobulin (IVIG) commercial only
- Infusion catheter care
- Inotropic therapy
- Iron binding therapy
- Pain management
- Steroid therapy
- Total parenteral nutrition (TPN)
E-Medical Supplies
E-Medical Supplies is the preferred provider for Cal MediConnect plans for diabetic supplies, such as:
- Blood glucose meters
- Blood glucose strips
- Lancets
LogistiCare
LogistiCare is the preferred provider for Cal MediConnect plans for all covered, non-emergency medical transportation services for Health Net members.
